The killer (Billy, or Buffallo Bill) wants to undergo a male to female sex change but he's been refused by a fair few hospitals so he's adopted a DIY approach. He kidnaps women who are around a size 14. He keeps them in a well in his basement, starves them for three days (so their skin is loose), gets them out of the pit, skins them and uses their skin to sew himself an all-over body suit to wear. The lotion scene you're talking about involves him looking down into the pit at the girl (I think her name is Catherine) and telling her to put the lotion on her body. He does this because he wants the skin to be in the best possible condition when he turns it into a suit.

This is really going to wreck the film for you if you do watch it but here's what happens in the end.

For most of the film it doesn't look good for Catherine but it finishes with Clarice discovering where she is kept and shooting Billy - which is a really good scene, seriously, you're missing out if you don't watch this film - and calling in the police, medics etc. and Catherine is saved.
The killer was a certified nut-job. One of the creepiest scenes in the movie.
Girl is in the pit- the killer says "it rubs the lotion on the body or it gets the hose again" or something like that over and over until she puts lotion on herself- otherwise he is gonna spray her with a fire-hose-
He's trying to get her to keep her skin moist because he wants to make himself a "woman suit" out of it.
Good movie. You should watch it.

Very creepy. The killer assumes the guise of a girl by wearing make-up and pushing his private parts between his legs...
Possibly as disturbing as some of the scenes from a clockwork orange.
she has to rub lotion into her body so the skin is subtle for when he cuts her skin off her and makes him a woman's costume out of female skin to wear

as he wants to be a woman
all of them are right...it's really creepy!
the lotion helps make the skin more pliable. easier to make a human "costume" with the flesh from her back.
