Costello says: "Years ago we had the church. That was only a way of saying - we had each other. The Knights of Columbus were real head-breakers; true guineas. They took over their piece of the city."

The Knights of Columbus are still a Catholic organization, therefore any nationality can join, just as long as they are Catholic. Italians are predominately Catholic as well, and by Costello's comments he is implying that the Italian Catholic population in Boston had risen so high that they took over the leadership and control of the Knights of Columbus in that city, despite the fact that the city and the organization are predominately Irish. The Irish community could no longer depend on or look to the KofC because of this, therefore, they only had each other. Boston had a large influx of Italians and Italian-based crime in the area during the time period he is discussing.
