He is not dead, but barely alive and living the fantasy. The Spanish verision is clearer on this and in many ways better. The original is called Abre los ojos, which translates to open your eyes. I saw it with subtitles but you can get it dubbed. See about renting it, it's a sadder, more touching movie.
