Question: Yeah. That commerical/magazine ad with Robert De Niro and Ellen Degeneres?
They list things and then at the end they say their card. I can't explain it, so you have to have seen this to help me.
Well, I need one of those forms for a school assignment, but can't find it ANYWHERE. Does anybody know where to find one or what the things are?

Sometimes you can find those ads in popular magazines...

I read a lot of mags and I haven't seen one in a while. You might want to try a used book store where they resell mags. I'm pretty sure Cosmo had them.
