Format for a pop culture Quiz?!

Format for a pop culture Quiz?

I am looking for the types of quizzes they have in teen magazines where the quiz is in a type of flow chart. You answer each question and follow the path as you go. I need the format of one, but can't find any online. If anyone knows any issues of magazines that are currently out that has one of these, can you let me know which mag and issue? Or, if you find a website, let me know!
