It is a sex reference. It doesn't necessarily prove insanity. It was just a way to make some money back in that time period. Granted, she did poison herself and that was implied that it drove her out of her mind.

To respond to the second part of your question...I am not sure that those lyrics actually made it to the movie. I know that some of the songs were taken out of order. I also think that far thinly veiled sexual references get ignored by censors.

Don't know the song - or the context - but the sex references are explicit enough for anyone to grasp. The choice of words would appear to be quite deliberate - and 'suggestive' of someone subject to serious abuse. Perhaps even to the point of being driven 'insane' - and of course we know the dividing line between sanity and insanity can be wafer-thin - when we are on the very edge of human endurance.
