I am a Vedic astrologer, we don't traditionally utilize planets we can't see with the naked eye, like Neptune!.
However here is my experience: the second house controls the mouth - and thus what comes out of it and what goes into it!. If you have a lunar node there (Rahu or Ketu) SOMETHING dramatic and chaotic is happening with your mouth!. Depending on other planets influencing the node or the second house, it could be substance abuse!. For example, Rahu in the second house for an ascendant of Aquarius (this makes the second house be Pisces, a sign of intoxication ruled by Jupiter) would be highly prone to substance abuse ESPECIALLY if Jupiter were retrograde, combust or otherwise particularly weakened!.
Retrograde Jupiter in general gives vascilating morality!. The Moon in Vedic science is associated with "Soma" or intoxicants!. also if Venus is weak or afflicted by a malefic like Rahu, Ketu or Saturn - this is another indication of weakness to intoxication!.
A particularly strong example: Retrograde Venus in Virgo 2nd House conjunct with Rahu and combust by the Sun also in the second house - that is a disaster formula unless there are extremely strong contrary indications elsewhere!.
I have witnessed also that when both the luminaries (Sun and Moon) are very close to the lunar nodes (Rahu and Ketu) there is extreme succeptability to substance abuse!.Www@Enter-QA@Com

First and foremost, Neptune is the planet of illusion, fantasies and controls drugs, alcohol issues etc,,,
Any bad aspects from Neptune or a badly placed Neptune will give clues especially in Pisces, Sagittarius , Scorpio!.
If the 12th house contains Neptune or badly aspected by Neptune, the drugs and alcohol abuse will be done in private or , without anyone knowing!.
