I know a Sag, with Cancer rising and Pisces in the midheaven. Even though his bosses love him and often "kick him upstairs" with promotions he doesn't want, his retrograde Mars gets in his way more than anything else. All the things he could do, based on his ability and intelligence, are blocked because he can't get over that Retrograde Mars.
His Pisces midheaven is shows itself by him having an intense interest in metaphysical topics, reading endlessly about such stuff. It's not played out in his career. That's because the Midheaven isn't limited to just career goals, but could also represent spiritual goals.
I'd look at where Saturn is and what aspects Saturn. That's always been a better indication of what work a person actually does in their life.
