It's basically a prequel to the movie Ichi the Killer (Great movie, though probably the bloodiest, goriest horror movie, I've ever seen!. Miike is a genius)
There aren't as many deaths as in ELfen Lied, but it really goes into what the mentality of Kakihara and the enjoyment (mainly sexual) that he gets from torturing, and killing people!.

I didn't watch this anime because I read the description and it was horrible but towards the end sombody cuts open somone's stomach to see if there was a fetus in there and sombody's head gets chopped off and a girl carries it around in a bag and talks to it o!.O

Hands down Elfen Lied!. MY GOD!. 23 people are butchered the first 10 minutes of the first episode!. They definatly don't hold back in the later episodes either!.
I couldn't believe my eyes at first when I began watching it!.
That's not my type of anime so the only other one I can think of is Vampire Knights!. Though is not kill Kill KILL! blood shows up frequently!.

Battle Angel Alita, Blood the Last Vampire and Elfen Lied are the only really bloody anime I've watched!. I'm more of a comedy anime person!.Www@Enter-QA@Com
