Yes he's been dead before the series fully began!. Minato Namikaze, 4th Hokage, died while sealing the 9 Tailed Fox inside Naruto!. The seal takes the life of whoever uses, so it caused his death!. His character is never actually in the series!. He may have been in the first few minutes of episode 1, but that's it!. Any other time would have been his face carved into the Hokage Mountain, the flash-backs of other characters, or speaking in reference to him!. The seal that killed him was later banned, but used by the 3rd Hokage to seal Orochimaru's arms!. The seal also took his life when he used it!.

yondaime was a nickname because he was the 4th Hokage!. Yon is four in japanese, so he was known as Yondaime!. Tsunade is also called godaime because Go is 5 in japanese and she is the 5th Hokage!.
