in the manga, it is stated that mayuri was classified as dangerous and was quarintined and locked up i the detention center in soul society that the 3rd seat of the 2nd squad who was urahara at the time looked over!. when yourichi made him a captain he mayuri free to be his asistant in the 12th squad!. when urahara let him out, he didnt understand why urahara let him out and questioned his thinking!.
in other words!. he always was dangerous and never trusted anyone form the start!.

Because he probably wanted another chance to study Uryu!. I think he knew something about the entire quincy-Bounto thing and that they would eventually come to soul society!. Yes he's a bad captain because of is selfish obssesive need for knowledge!.
