1!. Ichigo's motivation for continuing to be a soul reaper was in episode five with the whole Chad/bird thing!. I think he also did it because the kid(birds) mom died and because he had a somewhat similar story to his he didn't want anyone elses family or friends to be killed like his mom was!. I also think he did it because he didnt want people to get hurt!.
Ichigo got his soul reaper powers from Rukia so he could save his sister, Karin from the hollow that pretty much attacked her!. He had to pierce Rukia's sword through his heart so he could become a soul reaper!
I believe it was Karin Kurosaki who got kidnapped by a hollow, but I dont really consider it a great kidnapping considering Ichigo saved her in like less than 10 minutes! But I know that Orihime got kidnapped atleast 2 times that I can recall, but the only one that I can recall at the moment is the one by the espada!.
2!. I believe that it was used on 3 or 4 times
3!. I think that he died from using his Hiten-Mitsurugi-Ryu a lot!. He died on Kaoru and I believe that Kaoru died shortly after!. But who knows! Kaoru did say that Kenshin was just tired or something like that so maybe they were just sleeping!

Hat and Clogs (okay I can't spell his name!.) The one that was kidnapped was either that bird thing or one of Ichigo's sisters!.

1!. I think ichigo wanted to continue since he saw that little kid's soul almost get devoured by the hollow!. He got his powers from Rukia when she stabbed him!. And I don't think anyone has ever gotten kidnapped by a hollow except for Orihime this one time in a filler episode!.
