Wow, that's like 8 questions!
1) Naruto's father died after sealing the Kyuubi [Nine-Tailed Demon Fox] within Naruto. Basically when he sealed the Kyuubi --> he was left fighting in the Death God for eternity [the technique he used to seal the Kyuubi]. As for his mother, nothing is known of her and she is pressumed dead since she is never mentioned and was not there for Naruto.

4) Well, I would believe that Naruto knows he has parents. I mean he's not the sharpest knife in the drawer -- but he knows that in order for him to be born he must have parents. He just thinks they are dead.
5) Naruto's father and the Fourth Hokage's name is Namikaze Minato. Naruto's mothers name is Uzumaki Kushina. Naruto has his mom's last name.

[Edit] I would just like to point something out in rainee~loves Full Metal Panic!~'s answer :) ---> Yondaime isn't a part of Naruto's fathers name. Yondaime literally translated means Fourth -- he's called the Yondaime Hokage [Fourth Hokage].
1. & 2.) I don't why his mother died since it hasn't been revealed yet. His father died because he saved Konoha from the Kyuubi using a fatal jutsu. But then his body was never found so it's speculated that he is still alive somehow.
