The tatoo was given to her by a Shiar death squad that was out to kill her. The Shiar orded the death of the entire Grey family, because they feared the Phoenix force would manifest in another member of Jean's family. They attacked at a family reunion and killed nearly everyone very quickly. Only Rachel and her grandmother, Jean's mother, survived. One of the squad members marked her with that tatoo while she was temporarily down in the fight. It serves as a marker for all who would hunt her, and I think they can track her with it, I can't remember. Anyway, I still wonder why they didn't just kill her if they had time to mark her with a tatoo.
There really hasn't been much of a follow up to this story, as no more death squads have come after Rachel or her grandmother. Then again, Vulcan is in charge of the shiar empire now, and he might have other priorities. Perhaps ironically, Rachel is currently a prisoner of the Shiar Empire. No word on if her execution is impending or not, yet.
