I think he already has a bad eyesight during the battle versus Sasuke probably 30-40% of his eyesight has gone bad. Anyway he doesn't use Mangekyou Sharingan as often as Madara Uchiha
we know its failing bad. Kakashi noticed that he could only use it for a small fraction of time before him and his partner had to leave when they visited konoha. Sauske on the other hand has yet to show his Mangekyou. doesnt mean he doesnt have it. just shows he doesnt need it. I think itachi is still pushing sauske to get stronger so sauske will defeat madara. I think he's been making him think he's evil from the get go to push him to be the most powerful uchiha ever
