i think they are just flattering them... cuz they don't actually do them cuz that would not be allowed as a high school club... if you watch you'll realize that they live to make the dreams of the girls come true... like in that episode where they helped that one girl get together with her would be husband before he moved off... in the one episode with the sakura dance thing.... and they mostly just help girls... like they say their tag line in the beginning of almost every episode... plus if they did have sex with the girls that would be hard for haruhi to do since she is a girl in the first place... LOL...

In a regular host club in Japan (as I understand it), the purpose is to have someone who really just flirts a lot with you and so on. There are, however, places that have escorts for hire (prostitution). I don't think this is the case in the story because this isn't a mature themed manga or anime. Basically, they only tease, and nothing more than a quick kiss on the lips, not even making out.
