that was actually a fan edit using pic from early shippuden and kakashi gaiden,It's been around for like 2 years.I thought it was real when i first saw it too.It's also just been revealed that yondaime's real name was minato,and had a child whith kushina(if i remember correctly) uzumaki. Actually, the page itself was taken from various chapters of Naruto.
1. Yondaime's face was taken from Kakashi Gaiden chapter.
2. The Akatsuki member taking off his hat was actually Deidara. It was during his infiltration in Sand, to capture Gaara.
Yondaime's face was attached to Deidara's body and was somehow reversed, looking real.
3. Kakashi's reaction was actually from the chapter when Naruto wanted to spoil Icha Icha Tactics storyline during the Second Bell Test.
4. Naruto's reaction was actually from the chapter when he met Sakura for the first time after the time skip.
