This is mostly all revealed in Chapter 367 in the conversation between Jiraiya and Tsunade. The anime has not reached this far.
The Fourth Hokage's name is Minato Namikaze, although many people refer to him as Yondaime - because that's the Japanese word for 'Fourth'.
Naruto and the Fourth look alike because in reality the Fourth is Naruto's father. The Fourth died after sealing the Kyuubi in Naruto -- it would only make sense that Naruto is his son, because as seen in the anime, people neglected Naruto because he was host to the Kyuubi, therefore giving people no reason to abandon their newborn child, forcing Minato to seal it in Naruto.
Naruto's last name is Uzumaki, because he has his mom's last name and not his dad's. His mom's name is Kushina Uzumaki, and that she was a ninja of the Land Of the Whirlpool (Uzumaki is a Japanese word for vortex or whirlpool).

the fourth is the one who put the "nine tails" into naruto. He also told the village to treat him nicely although this does not happen. Even though the fourth died as a hero, he wanted naruto to be recognized as a hero too. If naruto wasn't there he would not have a place to store the "nine tails"

It is revealed in an newer chapter that the third was in fact narutos father. Jiraya says the the fourth wanted him to take care of his son, ie naruto.

It is his son. Jiraiya revealed the name of the father and mother in the manga. The father is the Third. If you ask me it's the least surprising "surprise" in the series considering I suspected it from the first few episodes.
You mean the Fourth Hokage. The Third Hokage is Sarutobi, the old man (Konohamaru's grandfather.) The Fourth was named Minato Namikaze, and yes, he was Naruto's father.

Its true the fourth is naruto's father. people should start calling him by his real name which is Minato Namikaze. His mom was Kushina Uzumaki from the whirlpool village. there both deceased.
Those were actually theories before Naruto Chapter 367 (if my memory serves correctly) revealed who Naruto was: the Fourth Hokage's son. So the first theory was right. :) I did think that Naruto was the Fourth's reincarnation, too, but I'm still glad that he was Yondaime's son.
So far, the chapter wasn't animated yet, plus it only has reached about Naruto searching for a new member for his mission to retrieve Sasuke. We have to wait for a very long time to see it revealed in anime. :)
