How do you plan your schedule around unreliable employees?!

Question:

How do you plan your schedule around unreliable employees?

I work in a small office. Right now, the staff consists of the employer (who is usually in Court), his wife (who works her own hours), myself (the overworked and underappreciated paralegal) and the receptionist (who has called in "sick" 3 times in the past two weeks) I make my plans and appointments based on the assumption that people will be where they are supposed to be/when they are supposed to be! It usually isn't rigid, but there are times like today when I have an unbreakable appointment at a specific time (I'm using my lunch hour for it) Now, I am the bad guy because I need to leave for it and it inconveniences my boss. Any suggestions?
