The stories are mostly true, but the interactions we see are highly encouraged by the staff and nearly scripted. Vh1 recently had a show going called the Springer Hustle, which showed the writers and producers finding the stories, then meeting with the people and telling them some things to say and how to act. Not necessarily saying "Call her a b- and then go punch her in the face!", but at least saying "and how does that make you feel, do you think she is a b-, doesn't it make you want to punch her?" Basically just instigating situations.
