As far as can be told by anybody who can look past Marilyn Manson's lack of eyebrows and lipstick, he is NOT a "nazi follower".
What you, and everyone, should understand is that Marilyn Manson plays with Shock-value. What is more shocking than somebody parading around in a nazi-esque uniform in front of a giant stadium filled with loyal fans ready to bleed for him? There isn't much I can think of.
Another thing you should understand about Marilyn Manson is that he is not a simple man. His image, music, lyrics, art, and artwork associated with his band are not simple and very little can be understood simply by first impression.
Although Marilyn Manson isn't considered "wholesome" or "good", he does have a household name and that's the point. You can love him or you can hate him. You can be afraid of him or you could want to "save" him. In terms of fame, it's good that people are talking about him, still spreading rumors that were started a decade ago. Any publicity is good publicity.
In reality, Marilyn Manson is a just a man. His job is that of a musician. He has tastes and interests that aren't what people would consider "normal", but none of them malicious or dangerous.
In terms of some of the nazi-esque images or video you might have seen, a lot of it has to do with the fascism of entertaintment as well as the fascism of religious evangelists. It's either a ridicule or a statement. Again, it isn't simple.
